This portal offers local information, advice, and national support for individuals with a life-limiting diagnosis (cancer and non-cancer), their families, friends and professionals too.
If you have any worries regarding your condition and the impact this is having on you or your loved ones, this portal aims to support you with links to websites/professionals who can guide you on this journey.
If you would like to be referred to the palliative care team, please speak to your GP practice or consultant/nurse who will be able to advise you further re this process or you can contact the services yourself directly and make a referral.
