This portal offers local information, advice, and national support for individuals with a life-limiting diagnosis (cancer and non-cancer), their families, friends and professionals too.

If you have any worries regarding your condition and the impact this is having on you or your loved ones, this portal aims to support you with links to websites/professionals who can guide you on this journey.

If you would like to be referred to the palliative care team, please speak to your GP practice or consultant/nurse who will be able to advise you further re this process or you can contact the services yourself directly and make a referral.

  • The single point of access (SPA) 01472 256256 is a local service, a team of health and social care professionals can offer advice, information and access to support for you or someone that you care for. The service can be contacted 24 hours a day, 7 days a week
  • CARELINK 01472 312312 offers electronic solutions to support you or someone you care for in the home
  • Compassion in Dying making decisions and planning your care
  • ConnectNEL 01472 403403 a dedicated 24/7 phone line and an extensive database of services
